FIRST VISIT CHECKLIST
1. Schedule an Appointment
Schedule your first appointment online now! We often have same-day, evening, and weekend availability.
If you cannot find a spot soon enough be sure to use the waitlist feature on the bottom left of the page. You will then receive notifications when something in your availability opens up.
2. Confirm Insurance Coverage
Insurance coverage varies by practitioner, so please see the chart below to clarify which of our practitioners accept yours!
Our office staff will do their best to confirm benefits prior to your visit, please feel free to call for further clarification.
3. Complete New Patient Intake
After scheduling your first appointment, you will receive our New Patient Intake Form via email.
Please complete it no later than 48 hours before your appointment time.
4. Note Our 24 Hour
Cancellation Policy
We require 24 hours notice to cancel or change appointments.
A $75 charge will apply to any appointment changed or canceled with less than 24 hours notice, as this usually means that we are not able to fill the spot at the last minute.
No-shows and repeat last minute cancellations will be charged for the full cost of the visit & cannot be billed to insurance.
UNDERSTANDING INSURANCE
In Network Providers
We accept insurance for all modalities at Mandala and bill to most major insurance companies! However, each practitioner is independent and therefore not all practitioners take all insurances.
Please consult the chart above to confirm that the provider you are seeing is in your network.
Patient Responsibility
Benefits quoted to us are subject to your plan limits at the time of service, and patients are responsible for the cost of treatment in the event that insurance does not end up covering services. Your benefits may be also subject to a deductible prior to your copay or coinsurance.

We always recommend patients confirm their coverage for themselves, as we can only go off of information relayed to us by your coverage reps who can be incorrect.
Copay or Coinsurance
Most visits have a copay or coinsurance between $10 and $50 per visit. We are happy to check on your benefits for you, please call our office in order to do so or when booking your initial visit online it will request your information in the form.
